TeleiCare Membership Terms & Conditions

1. Membership Overview

TeleCare offers subscription-based telehealth membership plans. Membership provides access to virtual medical services subject to the terms below.

2. Membership Plans

Individual Membership – $99/month
Includes:

  • Up to 2 telehealth visits per month for one enrolled member
  • Care coordination and ongoing medical support

Family Membership – $199/month
Includes:

  • Coverage for up to 4 family members within the same household
  • Up to 2 telehealth visits total per month shared among enrolled family members
  • Care coordination and ongoing medical support

3. Visit Limits

  • Visits are limited to the number included in the selected membership plan.
  • Visits do not roll over to the following month.
  • Unused visits expire at the end of each billing cycle.

Additional visits beyond the included limit may require an additional fee.

4. Billing & Subscription Terms

  • Membership fees are billed monthly.
  • Subscriptions automatically renew each month unless canceled.
  • By enrolling, you authorize TeleCare to charge your payment method on file each billing cycle.
  • You may cancel at any time. Cancellation applies to future billing cycles and does not result in retroactive refunds.

5. Refund Policy

Membership fees are non-refundable once a billing cycle has begun.

6. Medical Services Disclaimer

Membership provides access to telehealth services but does not guarantee specific treatment outcomes, prescriptions, or medical decisions. All treatment plans are determined by the provider based on medical necessity and clinical judgment.

7. Eligibility

Family memberships apply only to members residing within the same household.

8. Modification of Terms

TeleCare reserves the right to modify these terms at any time. Updated terms will be posted on this website.
