Insurance & Billing

How do I schedule an appointment?

After completing payment, you will be redirected to a confirmation page where you can schedule your appointment through our secure scheduling system.

Same-day appointments may be available depending on provider availability.

Transitional Care Management provides follow-up care after hospital or facility discharge. TeleiCare contacts patients within two business days of discharge and schedules a follow-up visit to review medications, symptoms, and care plans.

What is Chronic Care Management?

Chronic Care Management supports patients with ongoing medical conditions through regular follow-up and care coordination.

Do you provide wound care?

Yes. TeleiCare provides telehealth wound evaluations and care coordination. An initial in-person evaluation may be required depending on clinical need and location.

Is my information secure?

Yes. TeleiCare uses secure, HIPAA-compliant platforms for telehealth visits and scheduling. We do not collect protected health information directly through our website.
