With a passion for compassionate, patient-centered care and over a decade of nursing experience, Caroline Pena, MSN, FNP-BC, is dedicated to improving the health and well being of individuals and families thorugh accessible, high-quality healthcare. As a bilingual Family Nurse Proactitioner fluent in English and Spanish, Caroline is committed to creating an inclusive environment where every patient feels heard, respected, and supported.
Caroline’s clinical background spans wide range of healthcare specialties including primary care, chronic care management, home health, hospice, telehealth, dialysis, wound care, telemetry, behavioral health and addiction. Throughout her career, she has cared for patients with complex medical and behavioral health needs while emphasizing dignity, education and prevention, and holistic healing.
As an independent Family Nurse Practitioner, Caroline has extensive experience managing patients with co-occurring medical and psychiatric conditions, subsstance use disorders, chronic illness, and acute healthcare needs. She has worked closely with interdisciplinary teams to provide comprehensive treatement plans, medication mangement, diagnostic interpretation, and individualized patient education. Her approack combines evidence-based medicine with empathy, cultural sensitivity, and a strong belief in empowering patients to take control of their health journey.
Caroline earned her Master of Science in Family Nurse Practitioner from American Sentinel University and became board certified as a Family Nurse Practitioner through the American Nurse Credentialing Center (ANCC).
