Telehealth Consent & Disclaimer
TeleiCare provides care through secure telehealth visits using electronic communication technology. Telehealth allows patients and providers to communicate remotely. While telehealth is convenient, it may have limitations compared with in-person medical care. In some situations, an in-person evaluation may be recommended if clinically necessary.
Appointment Cancellation & No-Show Policy
Appointments should be cancelled or rescheduled at least 24 hours in advance whenever possible. Late cancellations or missed appointments may result in a fee or may limit future scheduling availability.
Late Arrival Policy
TeleiCare appointments are scheduled to allow adequate time for each patient. If a patient arrives late to a scheduled telehealth visit, the visit may need to be shortened or rescheduled to avoid delays for other patients.
Membership Billing Policy
TeleiCare offers membership-based services that may renew automatically depending on the selected plan. Membership fees are non-refundable once billed. Patients may cancel their membership at any time to stop future billing.
Telehealth Clinical Limitations
TeleiCare provides medical care through telehealth services when clinically appropriate. While telehealth offers convenient access to care, certain medical conditions may require an in-person evaluation, diagnostic testing, or referral to a local healthcare provider.
Patients understand that:
- Telehealth visits may be limited by the inability to perform a full physical examination.
- Providers may recommend in-person evaluation, imaging, laboratory testing, or emergency care when clinically necessary.
- TeleiCare may coordinate care with other healthcare providers, hospitals, or facilities if additional evaluation is needed.
If a medical emergency occurs, patients should call 911 or seek care at the nearest emergency department immediately.
